This application is for a USA Triathlon license. USA Triathlon is the national governing body for triathlon in the United States and it is a member organization of both the United States Olympic Committee and International Triathlon Union.

A pro triathlete needs to sign this form and complete payment in order to participate in a USAT sanctioned event. By signing the form, athletes also acknowledge that they are subject to medical controls put in place by USAT, USADA, and or WADA. This includes a medical control urinalysis test at events sanctioned by USAT or at a random time or location.
